[Board] Online form for performance details

Based on some discussion with Jim Carter and Sheila, I have created a space
on the website for people to "register" to play for a concert.  This will
put the onus back on individuals for entering the details they want to see
in the program for Eisemann concerts, and will also help us ensure who will
be attending for other events without continual follow up.  This way we
will only have to follow up on the outliers instead of everyone, and the
roster will be an easy download by event at any time.

Please  take a look at the link (it's on the member info page just under
where you would log in to download music) and let me know what you think.
